Custom Materials Form

Please note that any materials that contain bodily fluids or improperly processed materials (such as antler that is not fully dried and cleaned) will not be used and will be immediately disposed of. We will not return these materials to you.

For international customers, any import fees, duties, or other charges incurred when sending materials to us are the customer’s responsibility. If these fees are not paid in a timely manner and the materials are returned to us, the materials may be sent back to the customer at the customer’s expense.
